Identification of “At-Risk Patients”
Eligibility:
They hold a current Medicare card or Department of Veterans’ Affairs (DVA) card
They live in a community setting
They are at risk of medication misadventure
Their GP confirms that there is an identifiable clinical need and they will benefit from an HMR
As part of our HMR service, we are able to assist identifying for practices patients who would meet criteria
Who qualifies for a Home Medicines Review?
Currently taking fiver or more regular medications
Taking more than 12 doses of medications per day
Have had significant changes made to medication treatment regimen in the last three months
Taking medications with a narrow therapeutic index or medications requiring therapeutic monitoring
Experiencing symptoms suggestive of an adverse drug reaction
Displaying sub-optimal response to treatment with medicines
Suspected non-compliance or inability to manage medication related therapeutic devices (inhalers / puffers etc.)
Having difficulty managing their own medicines because of literacy or language difficulties, dexterity problems or impaired sight, confusion or other cognitive difficulties
Attending a number of different doctors, both general practitioners and specialists
Recently discharged from a facility / hospital in the last four weeks
Formulation of Medication Care Plan
Once the review has been completed, our pharmacists have been trained to provide a concise summary of their report to referring doctors. These recommendations can be modified as seen fit and distributed to patients, or discussed at their next health care appointment.
Accredited & Experienced Pharmacists
Our pharmacists have specialist qualifications enabling them to conduct high quality reviews in the patients home
All pharmacists involved in the HMR service will collaborate with consumers, community pharmacists, GPs, medical specialists, practice nurses, community health workers, allied health professionals, carers and family members.
Medication Profiles will be formulated for every patient following a review and sent to the GP. A Medication Profile includes:
Demographic, relevant social history and/or personal information
Medication management assessment
Active list of medications, including prescription and nonprescription, complementary medicines, compliance aids, therapeutic devices and appliances;
Dose, strength, dose form, directions, route of administration and duration of therapy for each medicine;
When necessary (‘prn’) medicines and the frequency of their administration;
Short term medicines (e.g. antibiotic courses);
Recent changes to patients medicines
Summary of recommendations for the GP’s consideration;
Telehealth Appointments
Telehealth is a beneficial platform that enables:
Health education and preventative care for high-risk populations
Cost-effective delivery model bring healthcare to places and people that don’t have it
Appropriate interventions delivered at the appropriate time
