Home Medicines Reviews are aimed at improving the Quality Use of Medicines for eligible Patients who may be at risk of adverse events or need assistance in understanding the best way to use their medicines. A Home Medicines Review is provided by an Accredited Pharmacist in the Patient’s home.

Fully MBS Covered
The patient will incur no cost for a home review if they fit the MBS criteria
Telehealth
For patients where home visits are not practical (eg. COVID-19 restrictions)
Physical Patient Home Visits
Our accredited pharmacists will drive to the patient’s home for no cost for a review
7-Day Turnover Time
After our review, we will aim to return our report promptly. Urgent matters will be forwarded sooner
Medication Plan Consolidation
Our experts will create a summary and proposed medication plan, which can be edited by their GP
No Cost to GP
There will be no cost of this service to patient or GP
